In short: Most epilepsy is well controlled on medication. Where seizures continue despite two appropriately chosen and adequately dosed drugs, the epilepsy is termed drug-resistant, and that is the point to investigate properly with video EEG rather than simply adding a third medicine.

After a first seizure

A first seizure needs assessment but does not always mean epilepsy. What we look for is the description of the event — ideally from someone who watched it — along with an EEG and usually an MRI to look for a structural cause.

Record what you can: what happened before, how long it lasted, whether there was tongue biting or incontinence, and how long recovery took. That account is more diagnostically useful than any single test.

Drug-resistant epilepsy

When two well-chosen medications at proper doses have failed, the chance that a third will work is modest. At that stage, video EEG monitoring localises where seizures originate, and for some patients surgery or neuromodulation offers a chance at seizure freedom that medication will not.

This assessment is under-used. Many people continue on escalating drug combinations for years without ever being formally evaluated.

What should I do if someone has a seizure?

Keep them safe, cushion the head, do not restrain them and do not put anything in their mouth. Time the seizure. If it lasts more than five minutes or repeats without recovery, call emergency services.

Does one seizure mean I have epilepsy?

Not necessarily. Epilepsy is usually diagnosed after recurrent unprovoked seizures, or after a single seizure where investigations show a high risk of recurrence.

What is video EEG monitoring?

Continuous EEG recording alongside video over hours or days, so seizures can be captured and localised. It is the key test when epilepsy has not responded to medication.

Can epilepsy surgery cure seizures?

For carefully selected patients with a well-localised seizure focus, surgery can produce lasting seizure freedom. Suitability is determined by video EEG and imaging.

Can I drive with epilepsy?

Driving rules depend on seizure control and local regulations. Discuss your situation with your neurologist before driving.
